Our services include the following:
Comprehensive
Assessment & Care Plan

Coordination

Fee Schedule
Initial Home visit, assessment and development of plan $50 to
$150
(Dependent upon ability to complete interview,
travel, and evaluation tools used)

*There is no
charge for initial telephone consultation. However, initial
assessment and follow-up visits are billed according to fee
schedule. Clients will be billed a minimum of one hour for follow-up
visits and additional time will be billed in half hour increments.
We do not receive payments from Medicaid, Medicare, or insurance
companies. Fees shown are effective 6/1/06 and are subject to change
with advance notice to clients. |
