Ministries . . . our
mission is to assist seniors in growing old gracefully . . . and
with dignity.
We're happy
to provide family caregivers the resources and expertise to assist
them in their care giving responsibilities.
There are many
resources available, yet many times go untapped. We'll be happy to
bundle the ones most pertinent to your needs and assist you in
taking advantage of the many programs available. First, we ask you
to tell us a little about your self or the person you're providing

“Gracefully Aging with Community Encouragement” is why
GrAce was established. We are a new
type of service to help seniors and their caregivers. GrAce
is a nonprofit geriatric care management service. We
work to match available services to meet your needs. Our job
is to do the research and the work for you so that you can
focus on solving your eldercare needs.
Simply put, GrAce
matches you up with qualified eldercare service providers
based on some basic information that you make available to
us. Your specific needs are gathered through a brief online
survey, which you complete and submit to us. The results
from your "Needs Analysis" survey are then compared against
the provider information in our database. We will identify
the providers which most closely meet the needs you have
identified for us. You'll receive a detailed synopsis of
all the programs and facilities available and we'll forward
them the information that you provide to us.

hours you will receive e-mail containing information about the
providers that meet your specific needs. You can then directly
contact these providers, who based on your specific needs will
be prepared to help you address your eldercare needs.
GrAce
Ministries is also
available to provide ongoing assessment and coordination of
geriatric services for you through our geriatric care management
program. Or, we'll be happy to contact
these providers on your behalf to assist you in setting up services.
