Available To You
Caring for your elderly
loved ones can involve a wide range of services. And GrAce
can help you with all of them. Here's just a sampling of the
services available that GrAce
can link you with:
GrAce
stands for “Gracefully Aging with Community Encouragement.” The
services provided are based on the needs of seniors and their
families. Services can include: Geriatric assessment; Development
of care plan to meet the needs identified; Assistance in
implementing care plan; Coordination of services needed;
Aging-in-place home assessment with housing adaptation and/or
modification assistance; Encouragement and prayer; Home visitation;
and Reassurance telephone calls. Seniors are assisted in accessing
existing community resources to prevent duplication of services
including assistance with applications and eligibility requirements.
GrAce
also recruits, trains and matches community volunteers with
homebound seniors to provide services that are not available in the
community, to reduce isolation and promote fellowship.
Geriatric care management
services are provided by a trained geriatric care manager and
provided on a sliding scale fee system based on the senior’s ability
to pay. Volunteer services are always provided without charge.
